Everyone should have the opportunity to live independently with dignity, especially the older adult population. Aging can at times make independent living a challenge and thus, I provide case management support. I recognize and understand that independence is important to the physical and mental well-being of you. The ability to make your own choices throughout the day is a huge impact on how you feel about yourself. The loss of independence can be isolating and lead to feelings of hopelessness and depression.


Case management services with you will almost always include your support system, such as your family or those who are of a positive support to you.

  2. Engage and increase your support network/ family to meet your needs

  3. Guide you in making decisions that positively impacts you

  4. Locate services and resources in your community to help you live a positive lifestyle

  5. Make referrals for your identified needs and goals

***The older adult population I often work with are those who can make decisions on their own but are in need of directions.


I do not provide personal services such as bathing, dressing, cooking, feeding, incontinence care/toileting, mobility assistance, etc.
